Falls with and without injury in nursing home settings (or even at home or in hospitals) are common but preventable when proper interventions are in place to address this issue.
The purpose of this book was to implement evidence-based intervention of hourly rounds using a checklist to assess individual needs to reduce fall rates. At the time of implementation, the project site had high fall rates.
When implemented, the findings indicated that the implementation of hourly rounding with this checklist featuring the six Ps reduced the fall rate by 50 percent. When implemented, this intervention improves safety and communication with caregivers or nurses to reduce falls.
